Registration for the National Conference on Prostate Cancer 2005 is $75 per person. Meals, taxes and gratuities are the responsibility of each conference attendee. Lunch is not included in the price of conference registration, but concessions will be available for your convenience. With regrets, PSA testing will not be available at this conference.

Price of 4 day Conference Registration:
Registration is $75.00 per person x the number of people.
Syllabus (advanced purchase required): $5.00 per copy requested
Gala Banquet (optional), Saturday, June 18th: $35.00 per person x the number of people.

Confirmation
You will receive confirmation of your conference registration by mail if your registration is received by May 25th. If you register after May 25th, you will receive confirmation at onsite registration.

Cancellation
Cancellations will be accepted in writing by mail, if received by May 25th. Cancellation requests not received in writing by May 25th will be considered a tax-deductible contribution.
